Social Services
 
The Social Services Department works closely with residents and families in planning admission to the facility. Our social worker and nursing staff will meet with the resident and family prior to admission to complete an assessment which assists in planning for the individuals need. The main goal of the Social Worker is promoting resident advocacy. The social worker promotes residents’ independence and make sure each resident’s psychosocial needs are met within the facility. The Social Service staff will talk with each resident after admission to discuss goals and will invite them to initial and quarterly care conferences. At St. Joseph’s Rehabilitation Center we promote the residents’ right to be treated with dignity and respect, as well as the need to feel cared for, secure, welcome and appreciated. Social services also assist residents and families with discharge planning to help make it a smooth transition.
